Newcomers' Checklist

Aloha and welcome to QCBR! The following is a quick checklist of items that you will need to review and complete.

Fire Safety

  • Amendment to QMC Fire Policy 1: This fire safety regulation pertains to hearing impaired personnel and/or visitors. Please read the following amendment, print it and sign it.
  • Amendment to QMC Fire Policy 2: Before exiting thru the emergency stairs, the lobby area by the elevator must be checked for potential personnel or visitors stuck outside. The front door and elevators may locked due to the fire alarm, therefore preventing anyone from entering and exiting the lobby area.

QMC Assessment of Risk

  • If you are not considered a Queen's employee (e.g., summer intern, temporary visitor staying for several days, not hired via Queen's, etc.), you must fill out the QMC Assumption of Risk form. After completion, return this form to Mahea.

QMC Departmental Orientation/Training

UH/JABSOM Lab Safety Training

All of the training can be found at the UH Environmental Health and Safety Office. Please check with Mahea for the site. Return all certificates of completion to Mahea:

  • UH/JABSOM Lab Safety Training
  • UH Biosafety Training
  • UH Hazardous Waste Generator Training
  • If you are working with animals, complete the Vertebrate Animal Training

IT Services Request

The UH Tower 8 has a different network from the QHS network. Thus, special VPN access is needed for you to access QHS intranet. Before you proceed,

  • Download the "VIP Access" app by Symantec onto your cell phone. Once installed, open the app and locate the "Credential ID".

Next, reach out to Mahea so she can login to the QHS ServiceNow portal in the Queen's Intranet. She will need to submit the following IT request forms:

  • "QCBR Staff Access Request"
  • "VPN"
    • Under "Please select the appropriate level of VPN access", select "Cisco VPN – GeneralPurpose".
    • Under "Do you need to remotely connect to a Queen's workstation?", answer "No".
    • Under "Justification/comments", type in "My VIP Access Credential ID is xxx. When VPN is setup, please contact me via phone (808) xxx-xxxx".

QCBR Timecard

You will need to gain access to the QCBR Timecard to record your hours for each pay period. Reach out to Mahea so she can login to the QHS ServiceNow portal in the Queen's Intranet. She will need to submit the following IT request forms:

  • "IM – EDW Application"
    • Under "Request Type", select "Application"
    • Under "Which application(s)?", select "Enterprise Applications – Other"
    • Under "Specify Enterprise Applications – Other", enter "QCBR Timecard"
    • Under "Environment", select "Production"

QCBR Website

The QCBR external website is https://qcbr.queens.org and we have all staff members' contact information. Please email the following info to Reinhold Penner (rpenner@hawaii.edu):

  • Photo (see the QCBR Team page for examples)
  • Full name
  • Job title
  • Office phone number
  • QMC email address (preferred)

Floor Access Codes & Door Key

  • Elevator Access Code: The elevators are typically operational daily from 8am to 5pm. However, during evenings and weekends, you will need a special code. Check with Mahea for the code.
  • Key: During evenings and weekends, the lab and office doors will be closed by housekeeping. Thus, if you get locked out of your office, locate the emergency box and open it. Check with Mahea for the code.
  • Door Entry Code: Once you get to the UH Tower 8th floor, you will need your own access code. You must not use someone else's code to get in, nor out. If you have a guest with you while exiting the floor, have your guest enter the guest exit code (to reduce the chance of the guest catching you entering your code to let them out). To request an access code, please contact Mahea with the following information: your full name, your Queen's email address and a 4 digit code. The 4-digit code cannot be a date, year, repetitive numbers (e.g.1111), nor consecutive numbers (e.g. 1234).

Intranet Content

  • Familiarize yourself with all of the QCBR intranet pages (House Rules, IT Support, FAQ, etc.).
  • Review all sections in the lab you will be working with, such as LCMS.
